Core Services and Programs

​

Enhanced Adult Foster Care (AFC):

 

We provide 24/7 person-centered support in safe, home-like settings. Services include assistance with ADLs/IADLs, medication supervision within AFC scope, transportation/appointment coordination, and consistent routines that promote dignity, choice, and quality of life.

Specialized Residential Services (SRS) for IDD & MI:

​

For individuals with intellectual/developmental disabilities and/or mental illness who need enhanced staffing and structure, we deliver SRS in licensed AFC homes. Supports include individualized staffing patterns, positive behavior supports and BSP implementation, crisis/safety planning, coordination with CMHSP/PIHP teams, and trauma-informed, non-aversive approaches. We align plans with person-centered goals and document measurable outcomes.

Community Living Supports (CLS) for MI & DD:

​

Skill-building and community integration services delivered in the home and community. Focus areas include personal care, hygiene, meal prep, household management, money skills, social/communication skills, travel training, and meaningful community participation. Services can be one-to-one or shared, based on assessed need and authorized hours.

Autism Specialty Program (Residential/CLS):

 

Environmentally structured supports for individuals on the autism spectrum, including visual schedules, sensory-friendly strategies, communication supports (e.g., AAC/PECS), and behavior support plan implementation. We coordinate with families, schools, CMH teams, and BCBAs where applicable to ensure consistent goals and generalization of skills.

Traumatic Brain Injury (TBI) & Auto Accident Care:

​

Specialized residential and community-based supports for survivors of brain injury, including those covered under Michigan No-Fault (PIP). We provide structured daily routines, cognitive and ADL skill-building (memory, executive function, safety awareness), behavior supports, medication supervision within AFC scope, transportation, community reintegration, and caregiver education. We collaborate with PT/OT/SLP, neuropsychology, case managers, TPAs, and insurers to align authorizations, documentation, and functional, measurable goals—from post-acute transition through long-term stabilization.

Offender Re-entry Residential/CLS Services:

 

For individuals returning from MDOC or Federal Bureau of Prisons, we offer structured residential and CLS supports that prioritize stability, lawful reintegration, and independence. Services include life-skills coaching, appointment and ID restoration support, linkage to employment/education resources, transportation, and coordination with parole/probation and community partners.
